There's so many possibilities, she needs to talk to a doctor. BPD (Borderline Personality Disorder) comes to mind especially when you mention the mood swings, paranoia, and dissociation, but again nobody here is qualified to diagnose anything. It's nice that she has a good friend like you who cares though
