Seems simple enough, psychologists have a science of doing surveys, etc., but they certainly seem not to use it to find problems with their services and try to improve them.

Yep. When my T cut me off and left me to lay in my pit of hell, it was done in secrecy and silence. She certainly did not solicit feedback from me, nor did anyone else. So statistically and in larger context it was a non-event.
