I had those kinds of thoughts from childhood, of hurting someone I loved, and realized they were just OCD. I knew I'd never do that, and these were thoughts, not urges, and I learned to ignore them. They hardly ever bother me, and never last long now, for years. I doubt it would cause you to do anything in your sleep.
I agree, stress has a lot to do with everything going on with you that you mentioned above. I get stress headaches, and I have irritable bowel syndrome (IBS), which creates a vicious cycle: I get stressed OR the IBS starts, and then one causes the other, and it keeps going on like that. I often end up taking Imodium, to help for at least a little while.
