I write to T every day, just a small paragraph but my new pdoc the only time I wrote to him I was suicidal and I ended up in the hospital. However he reads what T has written. Old pdoc I use to write all my symptoms. your last paragraph is fine to give to pdoc.

As for your questions: Yes my symptoms show up immediately but it could be you were already in an episode and that's why you forgot your meds.
