no according to your post they have not set the hearing date yet, they want you to fill out some paperwork.
one of those papers is a release of information form. this paper gives them permission to contact your treatment providers. the other paperwork is for you to tell them about your disabilities.
after they have this paperwork they will send your treatment providers paperwork for them to fill out and send back to them
after everything on your side of this is done (giving them permission to contact your treatment providers, telling them about your disabilities and giving them proof of your disabilities) they will schedule a hearing for you.
