Dissociation isn't a diagnoses this is a term. Is there disorder at the end of it, OSDD for example? Research the DSM-5. It certainly sounds like that if you are forgetting personal information then you might need to let your treatment provider know , so that they can track your continuum of dissociation 9 times out of 10 he or she is already taking note to see if your dissociation is disorder or not, pathological or not....
