Do you have a therapist? Mine did my mental health form and I was approved in 11 weeks. Mine does have a PhD in counseling and I don't know if that matters in whether he is allowed to do the form. They didn't give me a choice; I sent them a list of doctors and he got the form and filled it out. Which is really good because my pdoc is really slow with that stuff. But it isn't always the pdoc.
