Please do yourself a favor. First, take care of you~ you are the priority. You see hon you can be no good to your class feeling as you do now. Talk to your supervisor and come clean with him or her. Your depression is an illness that no one can hold against you legally or professionally. Believe me when I say, we all have someone in our family who has some form of illness... your boss included. I don't quite understand about the hours to make up for the program. But as I said before, right now it is your needs that matter. YOU are the top priority. Everything else will work out one way or another and worrying about it only harms you more and nothing ever gets solved with worrying anyway. Do right by yourself. Talk to your boss and go inpatient.
