I would try a triptan or fiorcet. Botox has also been approved for migraines. Have you gotten an MRI?
I would never suggest opiates for headache issues, unless it is completely incapacitating, to the point where you would consider the ER.
Opiates can give you a rebound headache, and they are very strong and addictive. Most reputable neurologists would not give you an opiate as a maintenance med for migraines.
