My therapist told me that (So I assume it is true) and the sheet I have goes by the week to identity emotions. It has columns where you fill out on a scale to 0-5 how intense your emotion is and if you acted upon those feelings. There is a column for self harm, suicidal thoughts, other targets (like cutting, hitting, etc), substance urges, drunk abuse, then emotions such as jealousy, love, anger, shame, sadness, etc. and if you used skills to help you cope with those emotions. Then at the bottom of the page there's a section where you write how your day went.
