I've done this. When I've gone to the hospital I've taken detailed med lists, medical history written out etc. I haven't needed to do this with a pdoc but I would if I changed. My current one knows 14 years of me so I don't have to explain too much to her.
I've found it helps and nobody gets offended. Well, except for the dr who cut all my meds in half, refused to look at my list and didn't understand why I was not sleeping and competely agitated (this was a medical stay for lithium toxicity). He truly didn't get it. Otherwise though, good experiences.
