Thank you everyone for the feedback. I think I'm going to do that. The problem right now is I don't know when/if I'll actually be moving so it is hard to plan for. She also tentatively offered a phone session or two after I move so I don't know when a good time to give her the card will be.

I'm already dreading saying goodbye. It makes me emotional thinking about it now.
