I'm a University professor and my suggestion is that you get a note from the hospital simply indicating that you were hospitalized (no need to state the reason) from Day X-Z. That is all the documentation a professor needs in order to give you a short extension for health/medical reasons. If you are asking for more than a week or two extension, then I would go the route of involving disability and/or counseling services.
