I do relate to a lot of what you're saying. I work from home and much of it is intended to be self-directed. When I'm not sure what to do, I tend to get paralyzed and depressed, which makes it much harder to focus. So I waste a lot of time being unproductive, which makes the depression worse. It's a vicious cycle that I'm trying to break out of. I don't have any magic solutions but would be interested to hear what you have been thinking of.
