Yes, I do all the time. I think mainly of conversations I had and reword it to the way I like it but then I lose what actually happened and have no idea what was part of what actually happened and what I made up. This usually happens to things that I truly care about, which is awkward, but... I don't have any advice on how to get rid of this though sadly. But my mind now is confused on what is and isn't reality. Well, for certain things anyways.
