I always liked Optimism Online
Optimism Mental Health Apps for Self-Tracking as an app that I could use on my laptop or my iPad. It is customizable and I could even journal on it. I believe there is a way to print and/or share with your doctor.
Another one is Mood Tracker
https://www.moodtracker.com/ which has similar features but isn't quite as customizable. You just have to play around with them until you find the one that works for what you need.
