I would definitely see a psychiatrist for treatment with medication. SSRI drugs (Prozac, Paxil, Lexapro, Zoloft, etc.) can help balance your brain chemistry so you are not obsessing so much and your anxiety level can go down with them. Benzo drugs can help with the anxiety (Xanax, Klonopin, Valium, etc.) at times when it gets more intense.
I take a supplement that helps my OCD thoughts A LOT: NAC (N-acetyl cysteine- sp?). I take 3,000 mg a day when my obsessive thoughts are bad (mine come and go with hormones every month).
If you are already on meds, I would go back to the doctor and tell him/her about your recent symptom changes.
I would also see a therapist or psychologist for CBT (cognitive behavioral therapy) to work on the compulsions and stress management.

P.S. Just for some reassurance, you likely NOT pregnant and you are NOT dirty or bad. Sex is a beautiful, wonderful thing when shared between people who care about each other. The thoughts you are having are OCD thoughts, not reality, and they can be changed with proper treatment.
