Ah, I see. So kind of an inner dialogue which you vocalise?
A friend of mine often does something similar, I actually find it quite endearing, it's part of who he is. Often interesting conversations can spring from these 'side tracks'.
That said, I can see when you are having a polite functional conversation such as changing an appointment it would be TMI. I guess if you catch yourself doing it, just quickly apologise and move on.
