Hello Miniongrl78: I'd be hesitant to advise you with regard to this. Laws can vary from one state to the next. (Plus I've been away from this for a few years now.) I would suggest you contact your state Department of Employment / Job Service office & ask if this is even a legal question. It's not unusual to find questions that are technically illegal on job applications. It is technically illegal for an employer to discriminate based on disability. But, of course, it happens every day... sadly...
One way of handling this kind of question I've heard of is to write: "Will explain in interview" in whatever space is provided. Of course this can be problematic as well because it sort-of puts the pressure on if you do get an interview. You'll need to be ready with an effective answer.
It is important, I believe, to disclose disability at some point prior to actually starting work. If you don't disclose, as far as I am aware, an employer is under no obligation to provide reasonable accommodation. It's been a few years now since I've been actively involved with these sorts of things. But this, at least, is the way it used to be. I assume it still is. There again, check with your state's job service office to get up-to-date, accurate information. Good luck!
