I'm on SSD, I worked a seasonal job last month and informed the director, the team lead and maybe one other person just because we was working together. I didn't ask for accommodation, lest we have to turn the job site in education forum for DID/PTSD it just makes for a sticky situation. Hindsight, in this particular role, I would have trial and error probably will next time depend on the work situation is asked that the if classroom trainer to keep their thoughts positive, jokes positive. I would ask them to not run alone side the dialogue ( of alters) and my abilities. I would ask them not to bring up any alters, never mind how they know ...I would make sure I share that I'm not HIV positive just in case that was the scare I just thought about this the other month....If you disclosed on application it gives a list of and I was like nooooo you think!!!!! LOL.....I would ask them to keep it work related, although they are, certain word choices are too close the dialogue alters. I would ask for cubicle or desk not near people and one on one help as oppose to classroom lead training. I've give an example, say for instance I've clicked on an application to the next screen , say for instance I want to do the same thing if an alter is out and doesn't quite remember, this is one on one's I don't need a co-worker who is sitting next to me bringing up something that sounds as if she is ALSO commentating the error that doesn't remember and the co-worker isn't watching. I don't know what is worst the attention getter or the now I'm mistrustful the hyperawareness flashbacks, avoidness and causes further switching. Yea the co-worker is oblivious, this pattern continues. If all is well and we are working there is always the prosecuting type team leader that will come around testing what you know, like I haven't pasted all the test too even get here. Couple that with later, we are always switching and experiencing PTSD it almost isn't worth it.
If you don't have this as a part of your therapy set up then you might not need this accommodation I'm about to present before you nor above. I suggest.

Mobile phone camera with time and day
Quiet office space( unrealistic but maybe)

Sticky Notes
Pens
Pencils
Desk Phone should have record feature that you can listen to ask well
Save all messages
Extra time to write or type
Wrist pad
Leg rest
Knock request at the cubicle (post a sign)
A preview of meeting group meeting and phone conference so that team can prepare best as possible
Name tag visible so that no one make a mistake to work on switching and ptsd
Think of any office aid that isn't going to break the budget
