My first thought was that the patient-psychiatrist interaction symbolized the therapy relationship to you. Maybe your own, or maybe just in general, based on all your study and experience in the internship. Your screaming and looking for help perhaps mean that you don't always agree with the therapy direction that the staff at your internship take, or perhaps that your own therapist takes with you. Also, your inability to loudly ask for help may symbolize difficulty in asking others to fill your needs. You were able to ask in this dream, but just not very effectively. The patient (a part of you) is pushing sticks into his face, which sounds like SI. A part of you appears to be asking for help with that, just not that effectively right now.
When you next go to your internship, can you look for evidence of the toy's origins?
