As a longtime sufferer from sleep problems, I offer you my condolences. Without some decent sleep, nothing in your life will go very well. I wouldn't be so reluctant to take the lorazepam, if I were you. A benzodiazepine used carefully is not going to turn you into a drug-craving addict in a short time. I used librium for years and found it helpful.

Make sure you're getting that GERD properly evaluated. Recently I found out that what I thought was just heartburn was actually a bleeding erosion and caused very serious problems that impacted my sleep hugely. Get some basic bloodwork done, if you haven't had it done lately.

Maybe the sertraline will help you. Older antidepressants, like amitriptyline, have sleepiness as a side effect. That's what I've taken for years and my sleeping is not half as bad as it would be without it. Another antidepressant that promotes sleep is mirtazepine (Remeron.)

You are not being all that heavily medicated, even though you are taking several meds. Take what you find helpful, and don't feel bad about it. It might also be good if you got some therapy to try and address what sounds like a significant anxiety problem you are having.
