All you need is insurance name, BIN, PCN, Id #, and card holder name
I did this when my insurance card took 1 month to come in the mail. My pharmacy accepted my information.

Thanks for this info, it is really helpful, should've asked for it today, but I was told.since it is a Medicaid health plan well am plan through them that my policy number is the same on both cards, confused me a little but they were able to look me.up through my MIhealth number (Michigan Medicaid card) so if I don't have it by next week when I refilled my Lamictal (I'm a week off Got my Geodon last night no issues, but I'm a week ahead on my Lamictal), so here's hoping I can get it this week maybe, it takes 30 days for it to come, and I enrolled.I think on August 3rd. But thanks for the information it really helps.
