Yes, my experience with No. 3 was much the same - she'd arrange her schedule so she could give me an extra 15-20 minutes as much as possible. She wouldn't accept payment.
My psychiatrist, however, is prone to charge unexpectedly and inconsistently. If I texted her about medications she wouldn't charge - but has now started to do so. (I don't believe it takes her 15 minutes to tell me to lower a dose and by how much if I'm having negative side-effects and write that down on her chart.) I can't track her time, since I'm not there - my solution has been to stop texting her.
I would either start tracking the time yourself or insist on a more specific accounting.
