I always write notes for my appointments with my Pdoc. If I don't I forget things. I also keep a journal of how I'm doing and take it to appointments. I used to be really good about writing in it every day but now I usually write in it once a week and kind of summarize how my week went. It helps me keep my theoughts organized when it's time to go to the clinic.
