Have you thought about telling your old p-doc about the insurance problems you have been having? Similarly with your therapist. It might be that one or the other or both would be able to do something about that to help the insurance process along so you could still keep seeing them. Otherwise, they might be able to reccomend a new p-doc / therapist who has a better relationship with your health insurance provider.

I'd discuss that with your current p-doc.
Different p-docs can have different takes on the utility of medications. That being said, I don't think that any doc would simply not give you another prescription if you have been taking something long enough to have built up a tolerance such that you are likely to have a withdrawal syndrome if you suddenly cease taking it.
Basically... I'd just explain about your insurance situation. I don't expect there would be any bad blood under those circumstances.
