I was like this for many years so I do relate. I was told I "present" well. But my husband advised me to be fully open (which is easier when manic and not too paranoid) and just harder to communicate with anyone (phone, text, in person) when depressed
They can't help you until you can be more clear about how you are feeling. Hopefully your sister can go to an appointment and advocate on your behalf.
