There are a lot of mistakes in their notes, on average. Many cut and paste notes from other clients with intent to revise, but miss a lot or misstate things. Or they are in a hurry to leave for the day and do it half-assed. In other words, they are often sloppy.
I've seen "odd" written only in context, such as oddly dressed, odd mannerisms, odd in appearance etc. But odd isn't a good descriptive word for that sort of thing. It's better to use more descriptive words that describe what the oddness is. Like if someone was 'odd" in dress, it's more descriptive to write mismatched, wrinkled clothes, etc.
