Technically, the more common forms of lactose intolerance in humans develop around adulthood, so those people, as babies, were able to consume and digest human breast milk without a problem. However, there is a rare congenital form of lactose intolerance that is present from birth. This can be problematic as the infant can't digest the human breast milk. These babies must be fed a lactose free formula in order to thrive. In the past, before the advent of formulas and bottle feeding, or in parts of the world where babies are solely dependent on breast milk as their source of sustenance, babies with lactose intolerance wouldn't survive past infancy.

As for the appeal of large breasts, I think it's more so a genetic predisposition for attractiveness as our distant male ancestors would often seek the females with large hips and breasts to bear their children. The large hips would allow the woman to bear children efficiently without many problems, while the large breasts would allow for ample milk production. However, not everyone is hardwired to like large breasts, especially as we as a society have since invented other means for infant nutrition other than breast milk.
