suggestion teeth guards are not only prescribed. you can get them in stores where sports equipment, bikes, skate boards are sold. they usually cost less then 5 bucks if bought this way and you can mold them your self. they are just mold able rubber that you soak in hot water then put in your mouth over and around your teeth. the modeling rubber hardens into the shape of your mouth and teeth in just a few seconds to minutes. then you take it out. then you put it in any time you feel your teeth need protecting. this is what I did when I did not have insurance and a dentist that prescribed it on insurance billing.

now that I have dental insurance my dentist just ordered me one as part of prescribing other dental needs.

which leads me to my other suggestion contact your dentist and have him prescribe the mouth guard and bill your insurance.
