Can you investigate further about why she misstepped? (Safety expert in me comes out ) Was she wearing shoes? Is there a worn spot on the runner? If no runner, was that part of the wood slick? Is there hand railing there? (Why don't people use it?) Did she have her mind on other things?

If you can help her think more about why it happened, maybe she can prevent it from happening again? Just be sure she doesn't start taking the "blame" for it... nah...it just happened.
