I think with the forums, it's not really needed.
partly because of the report post feature (and if someone did spot something against the rules, they could report it)
and partly because of the trigger code, admins/ moderators sometimes edit posts to add trigger code/ remove stuff not needed in posts.
but someone else would have to comment on it being used in chat.

its my understanding that people already know when they, their self have broken any of the rules here, the moderators do send out warnings and information of what rule was broken. they just do it by way of PM due to the rule that discussing administrative issues is not for public discussions on the boards.
I think of it this way... no one is perfect, everyone breaks rules at some point , if it was public domain then there would be discussions everywhere on the boards slamming, flaming and otherwise not being support to the one that has broken a rule. this site is a support group for mental disorders.
what if on the giving side of the general membership of knowing every single wrong someone does here and you have someone in a suicidal depressed state of mind...
for example I am in a highly suicidal state of mind and the general membership was told by a public tally all the wrongs I have done. how would that help my depression and suicidal state of mind. it wouldnt. would you really want to be part of pushing that member over the edge by requiring all their wrongs to be tallied and posted somewhere in their profiles or on the boards for all to see
now on the receiving end would you like all your own wrongs to be tallied and posted in your profile or anywhere else for all to see, how would that help your own mental problems?
suggestion... any one cam pm a moderator at any time and ask them how they feel their posting/ supportive history is doing here. Anyone can not delete any of the moderators pm that they send to members privately when members break any of the rules. maybe the members that want to tally their own wrongs/ wrong doings here can do so on their own computers.
my own take why cause other problems by pointing out their wrongs, when this is supposed to be a supportive group...
my point I can see many many ways in which this idea can go oh so so very wrong in so many ways.
