True. I would say that if they they think of clients it is because something has come up that sparked those thoughts. I know as a teacher, I don't spend much time thinking about my students(in fact I very purposefully try to leave my students at work), but things come up that remind me of certain students at often very random moments. I suspect it's about the same for therapists.
