I use moodtracker.com
This has a place for moods, sleep, irritability, and anxiety.
It also has a place to journal.
I print this off my phone and give it to pdoc and tdoc at each appointment.

Another great app to check out is "balanced". This one has you list everything you want to do:such as go to church once a week, make bed every day, see a friend once a week, walk three times a week, etc. It pops the items up to the top of the list so you stay "balanced". This one has helped me develop some really good habits.

Another app I highly recommend is the "suicide safety plan". I used to have one of these on a piece of paper, but then thought...hey, the phone would be a lot better.
