I have a lot of guilt & self-loathing I carry around.

What I strive to do is to simply accept it. There is a Buddhist practice called compassionate abiding that addresses these sorts of difficult emotions. One does not have to be a Buddhist to benefit from it though. Here's a link to a mental-health-oriented description of the practice:
