I've never come across anyone who does treatment plans except for one psychiatrist in the past. In that instance, a treatment plan was something we just quickly did to sort of get done because it was required to be in the file. It didn't seem to have real meaning.
I think it's a good idea to tell your therapist what your goals are, but I don't think a formal treatment plan is necessary for the average person. Might be helpful for minors (so that the parents can agree) or people who have guardians.

Since my goals are changing my treatment plan might as well, not sure I've ever been told I have a treatment plan though.
