I am so sorry you are feeling so bad. What about meds? Have you tried that or just behavioral therapy? Is there any way you could try a different therapist? Finding the right person really can make a difference. But I don't know if that's possible for you and if it is I know it's hard to take the energy to do things like that when you're depressed.
