Ok - so first thing I would ask - besides your lack of concentration, do you have anything else going on? Emotionally? Psychologically? Socially? Physically?

Negatively speaking, I am always frustrated or irritated (this is pretty consistent), usually tired, and since college, I tend to spend most of my time alone. Additionally, I have a pretty awful problem with procrastination or mere lack of motivation when it comes down to primarily schoolwork. I attempted extracurricular activities at my university, but I disliked them for the most part.
Positively speaking, I am significantly less tired than I was while in high school (due to more sleep, I suppose). I do still have some pretty close friends, though I do not really hang out with them as much anymore. I was never particularly fond of mere socializing, so most of the time I do spend with friends, I prefer actively doing something.
I usually have little to no issues in terms of frustration or any sort of foul mood towards wintertime, but the rest of the year it can become pretty bad. I do not have any history of actually lashing out at people in any form or fashion, so most of my frustration usually just becomes extra stress.
Physically speaking, I rarely feel actually ill. I do often feel tired, though my sleep schedule is somewhat inconsistent, which may cause a bit of it. My diet is relatively healthy, except for the fact that I tend to skip out on vegetables due to picky taste, and I do not exercise. Furthermore, my weight is in an appropriate range for my height.
