there are many suicide rating scales. any treatment provider (medical doctor, therapist, psychiatrist, nurse, er doctors) use it. it is just a few questions that are added in with the normal appointment to assess active suicide risk. here in NY its just a standard/ routine for treatment providers to ask these questions during yearly physicals or other routine appointments.

to get a copy for yourself to use on your own, anyone can contract their treatment providers. they will be able to supply you with a copy.

please keep in mind that not all treatment providers and states in the USA use the same assessment tool. your own treatment providers will tell you which assessment tool is used in your own state and by your own treatment providers....
