Forgive me if I'm just repeating other's...but here's my response.
You need to avoid the "most dangerous" mixed state. Go for the consultation BUT ONLY IF IT'S NOT TOO MANY CALANDAR DAYS AWAY and be totally honest. Be sure to ask how long it might take for recommended changes to kick in. See if there might be some medications that can be prescribed to make you more comfortable and less anxious until they do. See if some kind of out-patient program is available and if if that sounds necessary.
But if they recommend in-patient, take that advice. I understand you're worried about many things, number one being the burden this places on your wife. But how much MORE of a burden will it be if you get SO sick your illness causes a greater burden.
Finally, please remember that it's YOUR illness that's a burden on everyone. It's not YOU that's a burden.
Keep us apprised--

You are not your illness. You have an individual story to tell. A name, a history, a personality. Staying yourself is part of the battle.
