I don't know. I have one inside story I posted several years ago.

The professionals are individuals, and subject to the quirks of individuals.
The professionals don't work in a vacuum. They are almost always a part of some sort of bureaucratic medical system, which has its own quirks and imposes its own demands.
The professionals became professionals through education, which itself is a system.
