I think sometimes it can be hard to be believed when dealing with "professionals", like they think that we can't think for ourselves, or that we are the ones who make the mistakes.
How much detail do you need to give about the reasons for wanting to change providers? Can you say it just wasn't a good fit and wasn't helpful?

Yes, I think that would be the best option really. Maybe by my bringing it to her attention she will be more careful with other clients. But I certainly can't trust her now. What gets me is when they ask for honesty and feedback from us it's expected to be done, when you ask the same from them they ignore it.
If she had just talked to me about I may have stayed with her and given her another chance, I mean that was only our second time meeting.
Thanks for the feedback though. When I get the form I think I will just make it a bland response
