I've kept a journal for years. I find it very helpful to get my thoughts and feelings out of my head and to help me sort through them. But if writing isn't your thing and you just want a symptom log, you could try bullet journaling. There's loads of layout ideas online for tracking MH symptoms and sleep etc. I've not tried bullet journaling, but I'm considering it.

I'm sorry that the meds aren't helping with the voices. What coping skills can you use that have helped before? I hope that something helps.
