Someone correct me if I'm wrong, but in the vast seas of psychology literature, iatrogenisis seems barely covered. Most "ethics" literature is more about avoiding the appearance of impropriety -- a strong defense in lawsuits-- than understanding harm. One ethics book even subcategorizes the different-types of "hateful" clients i.e. clients with whom they've had ruptures. I can only speculate why practitioners seem to back off from the topic.
