I would include both diagnoses, as they both play a part.
Updated records are helpful to your claim, yet use whatever is available.
Get the claim filed, as the filing date is important. Should your claim be approved, the approval is retroactive to the filing date. (The same goes for associated "waiting periods," like the waiting period for Medicare coverage is retroactive to the filing date.)
