Honestly - there isn’t a test for ADD/ADHD. There’s a list of symptoms and it’s diagonisef by talking with you and or observing your behavior /talking to your teachers and parents if your still a child.

I’m honestly surprised that the center released that type of record to you. For example, I have ADD and a learning disability so I went through testing for larbing disabilities and the documentation I received included also included my ADd diagnosis but it was strictly related to my educational accommodations and didn’t include any sort of counseling notes which is what it sounds like is included in yours and is very uncommon to be released to a patient.

You asked about having it removed from your “record”. Not really. That office will always have it, you might be able to speak with another doctor at that office during an appointment and see if they’re be willing to add additional notes to it but they’re not going to remove the comments. Who do you currently get your medication from? Can they not just give you the documentation you need since it sounds like it’s not this clinic?
