I call MD's doctor X. And just X when I talk about them. I think it's pretentious for non- MD's to call themselves doctor--there's only about a billion of them in any (stupid) subject you can think of. If they prefer doctor I call them nothing to their face. When I was in graduate school we called our teachers Professor X, which was considered a higher status.
