I can totally relate to what you're describing here.
When you say "ADA violations" - do you mean that you've disclosed your diagnosis (whatever it may be) with your employer?
Also are you going to request a new boss? I was extremely frustrated (and scarred) by my attempt to talk to HR about my issues. My boss was a cruel, over-promoted, underqualified workaholic who routinely overpromised and underdelivered and blamed others for his inability to deliver. He made my life miserable, especially after I disclosed to him that my wife and I were going through an acrimonious separation (thankfully my wife and I are back together again).
To be "proactive" and get everything documented, I contacted HR. Amazingly, the jerk was able to convince them that *I* was the screw up, and that everyone else shared his view that I was incompetent. I was put on a "corrective action" in November, then an "elevated corrective action" in April, before getting canned in July.
Granted - my recollection of events is hazy at best. I was in the deep throes of a full-blown manic episode. Sleeping 3 hours a night, smoking incessantly, engaging in very risk-seeking behavior…basically every bullet point of the DSM-V diagnosis for bipolar. So while my boss was a complete jerk, I am sure many of the punitive actions were merited. However - I strongly advise against consulting HR trying to get a new boss. More than likely, the firm will side with the abusive boss - they are, after all, there to protect the company's interest, not yours.
So, I wish you all the best. Your experience may be different from what I experienced. However, when working with HR I would strongly advise to proceed with caution.
