I've used CBD and as TMC says, the total milligrams refers to the whole bottle. I also found it confusing. It takes a bit of math to figure out dosages in drops, but then, I don't think there are clear guidelines about dosages for mental health purposes yet. The half-life of CBD is a few or several days, so, when you start using it you're kind of building up your levels for the first week, maybe. So, to give it a fair trial you might choose a dose that seems affordable and stick with it for a week or two before deciding if it seems helpful.
When I've used it (about 10 drops a day of what I suspect is a similar concentration to yours) I noticed very little effect for the first few days, but I eventually slept better, and felt appropriately tired and sleepy when I should have been tired (instead of being keyed up or wired late at night). It may have helped a bit with anxiety too, though not as much as other things I've taken (valerian root, chamomile).
