What have you tried for akathisia? Two really mild, benign medications help me with it: Gabapentin or Atarax. Both also help with sleep. Oh, sorry, I see that you are on Gabapentin already. Atarax is a strong antihistamine. You do not list the past meds you have tried so I cannot tell. I did not know that antiparkinsonian agents can exacerbate TD.
